A B S T R A C T

The development of technology and information systems at present occupies a key role and is needed by a company or organization. This is realized by using the computer as a tool that is able to store and process data in a rapid, precise and accurate. Along with these developments, a company or organization required to improve the quality of information systems in order to compete in the era of globalization. To improve the quality of information systems, the optimal use of technology will support the efficiency and effectiveness of work, so as to produce an accurate output.

PKU Muhammadiyah Yogyakarta Hospital has to apply information technology to improve the quality of care for patients. Radiology Installation PKU Muhammadiyah Yogyakarta Hospital is one of the installations in the PKU Muhammadiyah Yogyakarta Hospital being developed computer-based information systems to support their operations. Previously, the staff at the Radiology Installation PKU Muhammadiyah Yogyakarta Hospital using spreadsheets applications such as Microsoft Office Excel for data processing. The growth of patient data and examination of patients in the future be considered for Radiology Installation PKU Muhammadiyah Yogyakarta Hospital to develop an information system administration in an integrated examination of patients with the database system.

The information system of examination administration in Radiology Installation PKU Muhammadiyah Yogyakarta Hospital was developed by the Unified Process. Unified Process is included in the paradigm of object-oriented software development. The system is implemented using the PHP programming language and MySQL as the DBMS. This information system can help the radiology staff in managing patient administration data inspection and report on the administration of Radiology Installation.
